This form is required for all new clients interested in a haircut, color, or both. It helps me determine if your desired look is achievable and if we are a good fit for each other.

I am NOT accepting clients with any box dye or color from the drugstore, Sally’s, etc., or anyone who has had professional RED or BLACK permanent color. Please do NOT fill out this form if you’re looking to go lighter and have any previous box dye or dark permanent color in your hair. (Toners are okay!)

Deposit Notice

A $50 non-refundable deposit is required to book your appointment. This amount will go toward your service.

Cancellation Policy

If you cancel or reschedule within 48 hours of your appointment, your deposit will be forfeited and cannot be used for another service. If you reschedule with more than 48 hours notice, your deposit will be applied to your new appointment.

Schedule Information

My schedule is Tuesday through Saturday. If you would like a Sunday or Monday appointment, there is a higher rate as those are my days off.
